Neighbourhood Nurse Practitioner
Key Responsibilities
- Work as a member of a self-directed neighbourhood nursing team delivering person-centred integrated care in the community.
- Support people to remain independent at home, proactively facilitating self-care.
- Focus on improving self-reliance and empowerment, considering patients holistically and supporting their health, social, and psychological needs.
- Provide consistent care to a defined community caseload of patients 24/7, 365 days a year.
- Share designated responsibilities within the team and rotate specific roles.
- Support the ethos of a self-directed team, making decisions about workload, recruitment, and training.
- Collaborate closely with GPs and other healthcare professionals, including therapists, mental health professionals, social workers, and case managers.
- Work in support of the teams core aims of happy patients, happy nurses, and care givers.
