Cancer Support Worker
Key Responsibilities
- Support implementation and delivery of Personalised Care for cancer patients.
- Conduct holistic needs assessments (HNA’s) with patients.
- Develop and produce care and support plans.
- Provide a single point of access for urgent or specialist needs.
- Offer health and well-being information and support related to cancer.
- Guide patients to supportive services.
- Empower patients to manage their cancer.
- Collaborate with the Macmillan Information & Support Service for referrals.
