• Planning, coordinating, and delivering Health and Social Care activities at Hartcliffe City Farm (HCF).
• Developing new Health and Social Care services at HCF in response to local needs.
• Gathering data for reports.
• Processing new referrals.
• Carrying out participants risk assessments.

Skills and Expertise

• Passionate about supporting adults and improving health and wellbeing through farm-based activities.
• Experience in health and social care (preferred).
• Administrative skills.
• Project management skills (preferred).
• Knowledge of risk assessment procedures (preferred).
