Team Leader – Supported Living – Stoke-on-Trent

Key Responsibilities

  • Providing support to Supported Living Services.
  • Ensuring person-centred support.
  • Overseeing the smooth running of services.
  • Completing regular audits, checks, and supervisions.
  • Updating and reviewing support plans.
  • Supporting individuals to achieve their outcomes.
  • Scheduling and allocating work to Support Workers.
  • Assisting with medication management.
  • Supporting access to community resources and finance management.

Skills and Expertise

  • Previous experience working with individuals with Learning Disabilities, autism, and mental health issues.
  • NVQ Level 2 in Health and Social Care (with willingness to work towards Level 3).
  • Excellent written and verbal communication skills.
  • Willingness to learn in line with client needs and expectations.
  • (Previous Team Leader experience advantageous, but not essential)
