Key Responsibilities
- Implement person-centered support plans, promote independence, manage risks, encourage relationships, and ensure legal and best practice compliance.
- Develop flexible, engaging activity plans that promote autonomy and personal growth.
- Support individuals in maintaining healthy lifestyles and managing their health needs.
- Facilitate effective communication and maintain detailed, person-centered records.
- Oversee medication processes and support individuals in self-administration.
- Maintain a safe, clean-living environment and adhere to health and safety protocols.
- 4 nights a week and personal care is involved.
