Support Worker – Complex Care Derbyshire
Key Responsibilities
- Helping people create and follow their Person Centred Plan
- Supporting relationship development
- Enabling informed choices about daily life
- Promoting self-advocacy
- Supporting full and purposeful lives in the community
- Encouraging social activities and relationships
- Assisting with finances, medication, practical tasks, and personal care
- Acting as a point of contact for families and social workers
- Reporting and recording care and support issues
Skills and Expertise
- Effective communication with people using services, families, and colleagues
- Understanding of learning disabilities and related issues
- Experience supporting individuals with challenging behaviours
- Relevant certifications and mandatory training (Basic Life Support, Manual Handling, First Aid, Oliver McGowan, Prevent)
- MAYBO trained
- Medication trained
- Excellent written and interpersonal communication skills
- Compassionate and empathetic approach to patient care
- Ability to work independently and in a multidisciplinary team
- Reliability, punctuality, and dedication to high-quality care
- Enhanced Adults and Children’s DBS (update service)
- Minimum 6 months experience in a Mental Health/Learning Disabilities setting
- Full UK Driving Licence
