Support Worker
Key Responsibilities
- Provide person-centred support, as detailed in service user care plans.
- Facilitate groups and activities.
- Work 1:1 with service users.
- Act as key worker, including facilitating IPP meetings.
- Collaborate with the Support Coordinator to maintain updated support plans and risk assessments.
- Communicate effectively with service users, family members, carers, other agencies, colleagues, and managers to ensure consistent and high-quality support.
- Work under the direction of Support Coordinators and the Day Service Manager.
- Perform physically demanding tasks involving significant time spent on your feet, supporting service users with diverse activities and personal care.
Skills and Expertise
- Previous experience in a similar social care setting with a working knowledge of adults with learning disabilities and/or autism.
- Person-centred approach to supporting adults with learning disabilities.
- Functional Literacy and Numeracy level 2 GCSE grade C/4 or equivalent (including English Language and Mathematics).
- Care Certificate qualification or commitment to obtaining it.
- Knowledge and understanding of safeguarding principles and policies.
- Ability to work independently and as part of a team.
- Strong communication skills and the ability to build relationships.
