Social Worker – Adult Community Mental Health
Key Responsibilities
- Assess and support service users to achieve positive mental health recovery outcomes.
- Support service users’ carers and families.
- Ensure Health and Social Care Partnership meets statutory legal responsibilities (Adults with Incapacity, Adult Support and Protection, Self-Directed Support, Carer Support and Involvement).
- Evidence commitment to and promotion of social work values within an integrated model of care.
- Manage a caseload, assess care needs, develop, implement, and evaluate care provision.
- Participate in the continuing development of the service.
