SALARY: £26,457 per annum + £3,000 London Allowance + Excellent Benefits

LOCATION: Field Based across Multiple locations including the Boroughs of Brent, Harrow, Hillingdon and Westminster

JOB OVERVIEW

We have a fantastic new job opportunity for a Community Mental Health Recovery Support Worker with experience of working in Mental Health and/or Social Care, good level of written literacy and numeracy and the ability to work independently.

As the Community Mental Health Recovery Support Worker your main responsibilities would be facilitating discharge from step down and coordinating the support needs of the patient. You will help organise and coordinator the care in stepdown for each patient, and you will recognise early identification of factors that would potentially delay timely discharge and impact on provision of appropriate aftercare.

Working as the Community Mental Health Recovery Support Worker you will liaise with housing and local authorities around discharge and facilitation of housing assessments. You will be responsible for coordinating the support for the patient allocated to you which can include ADL support, escort to appointments and arranging recovery activities.

The Community Mental Health Recovery Support Worker will take a role in embedding recovery values within the service and MDT Team, alongside other Trust recovery champions, and act as an ambassador of recovery for the Trust with external agencies and partner organisations.

CANDIDATE REQUIREMENTS

Essential

• Experience, paid or unpaid, of working in Mental Health and Social Care

• Education or qualifications in health and social care, mental health care or working towards

• Good knowledge of working with people presenting in mental health crisis and their families/carers

• Experience of using formulating plans to support individuals to maximise their independence and work towards their recovery

• Good knowledge of mental health policies and legislations that may contribute to support for patients

• Experience working with multidisciplinary teams, multi-agency workings

• Proficient, with the ability to problem solve and create innovative solutions to help empower service users

• Able to work independently

• Good level of written literacy and numeracy

• Able to prioritise workload and deliver service to a high standard

Desirable

• Have a good knowledge of local services surrounding the Boroughs of Brent, Harrow, Hillingdon and Westminster

• Have good awareness to respond in a professional manner to distress, disturbance and unpredictability

• Experience of using strength-based approaches to support individual to maximise independence and achieve their goals
