Mental Health Practitioner Band 6 – East Birmingham

Key Responsibilities

  • Providing mental health triage, assessment, and outcomes-based intervention for service users within the Neighbourhood Mental Health Team.
  • Identifying need and recommending the best course of advice, support, or intervention.
  • Promoting psychosocial wellbeing by identifying and supporting pathways for service users and carers.
  • Ensuring robust communication across mental health providers, VCSE organisations, and primary care, according to individual service user needs.
  • Providing support and advice to colleagues within the PCN as required, including participation in local neighbourhood MDTs.

Skills and Expertise

  • Proven experience within a multi-disciplinary team environment.
  • Strong communication skills.
  • Ability to work collaboratively with GP’s and other primary care professionals.
  • Knowledge of mental health care standards and procedures.
  • Understanding of community mental health principles.
