Mental Health Practitioner Band 6 – East Birmingham
Key Responsibilities
- Providing mental health triage, assessment, and outcomes-based intervention for service users within the Neighbourhood Mental Health Team.
- Identifying need and recommending the best course of advice, support, or intervention.
- Promoting psychosocial wellbeing by identifying and supporting pathways for service users and carers.
- Ensuring robust communication across mental health providers, VCSE organisations, and primary care, according to individual service user needs.
- Providing support and advice to colleagues within the PCN as required, including participation in local neighbourhood MDTs.
Skills and Expertise
- Proven experience within a multi-disciplinary team environment.
- Strong communication skills.
- Ability to work collaboratively with GP’s and other primary care professionals.
- Knowledge of mental health care standards and procedures.
- Understanding of community mental health principles.
