Key Responsibilities
- Delivering vital early intervention and support through the Healthy Child Programme.
- Working closely with local Neighbourhoods, City of London, and Hackney Children and Family Hubs.
- Providing personalised care to children and families, ensuring optimal outcomes.
- Acting as a named link to GP practices, Early Years Settings, and Nurseries.
- Managing vulnerable children by participating in multi-disciplinary team meetings.
- Guiding health promotion activities and key interventions (e.g., integrated development reviews using ASQ and ELIM questionnaires).
- Supervising the nursery nurse team’s health promotion work.
- Providing tailored advice to local children’s centres and parents to support children’s development.
