Health and Wellbeing Coach

Key Responsibilities

  • Coach and motivate patients through multiple sessions to identify their needs, set goals, and support them to implement their personalised health and care plan.
  • Provide personalised support to individuals, their families, and carers to ensure active participation in healthcare.
  • Providing interventions such as self-management education and peer support.
  • Supporting people to establish and attain goals.
  • Working with the social prescribing service to connect individuals to community activities.
  • Involvement in group activities, consultations, and support groups.
  • Provide support to local community groups and collaborate with other providers.
  • Facilitate groups of patients for consultations and goal attainment.
  • Utilise IT and MDT channels to screen patients.
  • Plan and respond to workload according to priorities.
  • Support delivery across wider locality areas.
  • Undertake any required training to maintain competency.
  • Take responsibility for self-development and on-the-job training.
  • Patient Care – Communicate effectively, using appropriate language.
  • Effectively use all communication methods and manage communication barriers.
  • Recognise and manage challenging behaviours.
  • Provide information to patients, carers, and relatives.
  • Supporting Care Delivery – Be the point of liaison for service users.
  • Interface with health and social care professionals.
  • Follow through actions identified by the MDT (including arranging tests, referrals).
  • Ensure all services and care arrangements are in place.
