Key Responsibilities
- Assess and manage patients in face-to-face settings, working within a multi-disciplinary team to provide high-quality, patient-centered care when normal primary care is closed.
- Provide timely, high-quality clinical care and assessments during base visits and support telephone triage.
- Deliver personalized, evidence-informed medical care, including advice, treatment, and onward referrals.
- Adhere to local prescribing guidelines and maintain accurate medical records, ensuring appropriate documentation and coding for service audit and monitoring.
