Extra Care Support Coordinator
Key Responsibilities
- Providing person-centred care and support to service users.
- Managing the operational management and quality of the service.
- Collaborating with Social Services and other stakeholders.
- Ensuring the highest care standards are met.
- Empowering service to meet client needs.
- Working with clients’ families and friends to promote a positive atmosphere.
- Maintaining client orientation and independence.
- Providing support with dignified personal care and domestic tasks.
- Supporting individuals to access work, education, and social opportunities.
- 2 days management in the office, 3 days direct support shifts (division can change).
Skills and Expertise
- Previous practitioner or management experience in services for older people, people with dementia and memory loss.
- Warm, person-centered, and respectful approach.
- Empathetic and holistic approach.
- Flexibility, energy, and reliability.
