Key Responsibilities
- Assessment, coordination, and delivery of high-quality mental health services for patients requiring secondary mental health services.
- Pathway planning, brief interventions (up to 12 weeks), crisis intervention in the community.
- Gatekeeping for inpatient wards and community recovery houses, with emphasis on potential home treatment.
- Ongoing service development to ensure relevant care pathways and plans for urgent and crisis care.
- Promotion and use of evidence-based practice through clinical advice, support, role modelling, and education/training.
- Liaison with carers, statutory and non-statutory services, emergency services, and 3rd sector services.
- Liaison with local services Single Point of Access for rapid response to mental health crises.
- Delivery of supervision for junior colleagues.
- Working within a 24/7 rota system ensuring comprehensive service provision and robust gatekeeping.
- Contribute to the assessment of physiological and/or psychological functioning.
- Contribute to planning, delivery, and monitoring interventions/treatments.
- Discuss assessments with the work team and understand individual roles in overall assessment and activities.
- Respect individuals dignity, wishes, and beliefs and obtain consent for assessments.
- Identify appropriate methods, techniques, and equipment for different activities, considering any risks.
- Undertake and record assessment activities as agreed with the care team, following established protocols and legislation.
- Report assessment findings appropriately.
- Offer insights into health and well-being needs and wishes of service users, suggesting needed treatments.
- Maintain awareness of Trust medicine management policy and protocols and ensure adhered to practice guidance.
