Community Physiotherapist – Home First – Hammersmith & Fulham
Key Responsibilities
- Support patient transition from acute to community services, providing community assessments for the Home First pathway.
- Partner with colleagues in acute, social services, and primary care to ensure safe, timely, and seamless patient transfer.
- Coordinate Home First HCA care delivery.
- Ensure effective onward transfer of patient care from Home First to community pathways.
- Identify/case find potential CIS patients in partnership with acute colleagues, ensuring timely transfer to community services once medically stable.
- Independently assess health and social care needs at home and plan required community services.
- Support and enable safe discharge from secondary to community services, bridging the gap from hospital to home.
- Promote a seamless and multidisciplinary approach to health and social care.
- Organise daily management of the Home First team to meet service requirements, ensuring access to equipment and resources for the HCA team.
Skills and Expertise
- Clinical expertise in physiotherapy or occupational therapy.
- Evidence-based practice and current best practices.
- Assessment, planning, implementation, and evaluation of interventions.
- Optimization of patient safety following discharge.
- Holistic and risk assessment of patient needs.
- Individualized care plan development and management.
- Excellent communication and interpersonal skills.
- Strong teamwork skills within a multi-agency environment.
