Community Occupational Therapist – Band 5
Key Responsibilities
- Deliver high-quality Occupational Therapy assessment and interventions as part of a skilled team.
- Collaborate with GPs, Adult Social Care, and healthcare providers.
- Engage in a range of training opportunities, including preceptorship, functional assessment, and moving and handling.
- Access regular clinical supervision for professional development.
- Utilize electronic records and work as a mobile occupational therapist.
- Hold a full and valid driving licence with car access for work purposes (unless exempt).
- Provide high-quality assessment and treatment packages in various settings (beds, homes, communities).
- Manage an individual caseload with minimal direct supervision.
- Actively participate in supervision, training, clinical reasoning, peer review, case conferences, and reflective practice.
- Represent the Occupational Therapy service and patients in multidisciplinary team meetings.
- Coordinate multidisciplinary services and integrate occupational therapy into patient treatment plans.
- Use and maintain all relevant equipment safely and competently.
- Organise and plan caseloads based on service and patient priorities.
- Adapt plans as needed to meet changing situations.
- Collect data for service audit and research.
- Participate in research projects related to clinical practice and service delivery.
- Support rehabilitation assistants/therapy assistant practitioners, and students on placement, as required.
- Liaise with various stakeholders (consultants, families, carers, multidisciplinary teams, etc.).
