Community Nurse – Hertfordshire
Key Responsibilities
- Providing skilled nursing care to patients in various community settings, adhering to best practice and clinical evidence.
- Managing chronic diseases, providing end-of-life care, promoting continence, and managing wounds.
- Performing phlebotomy, medication administration (syringe drivers, IV therapy), PEG management, urinary catheterisation, and tracheostomy suctioning.
- Acting as a Named Nurse, responsible for holistic health needs assessment, care planning, evaluation, and ensuring care continuity.
- Assessing physical, psychological, and social needs of patients and carers using Single Assessment Process.
- Creating individualised, person-centred care plans to promote self-care and prevent hospital admissions.
- Acting as a patient advocate, promoting independence and self-care.
- Identifying, recording new problems, and reporting to the DN Team Lead.
- Participating in the implementation and monitoring of Trust clinical and operational guidelines.
- Adhering to the NMC Code of Conduct and other professional guidelines.
- Ensuring safe handling of body fluids and contaminated sharps.
- Addressing the needs of vulnerable adults, including risk assessment.
- Developing and utilizing specialist skills in leg ulcer management, syringe driver setup, IV drug administration, continence assessment, and palliative care.
- Supporting patients and carers with medication management and reviews.
- Providing care to patients with long-term conditions, undertaking reviews per care plan with experienced colleagues as needed.
