Clinical Pharmacist Mental Health Services

Key Responsibilities

  • Deliver a comprehensive clinical pharmacy service to designated mental health wards.
  • Provide specialist information and advice to multi-disciplinary team members.
  • Complete medicines reconciliation following admission or transfer.
  • Record clinical interventions and prepare pharmaceutical care plans.
  • Work alongside teams to ensure safe and timely medicine supply.
  • Clinically check inpatient prescriptions, leave, and discharge prescriptions.
  • Provide specialist advice on medicine use to service users and carers.
  • Ensure cost-effective and efficient resource use across care interfaces.
  • Comply with formularies, guidelines, policies, and procedures.
  • Participate in clinical audit and practice research.
  • Report near misses and medication incidents.
  • Assist with education and training sessions.
