Clinical Pharmacist Mental Health Services
Key Responsibilities
- Deliver a comprehensive clinical pharmacy service to designated mental health wards.
- Provide specialist information and advice to multi-disciplinary team members.
- Complete medicines reconciliation following admission or transfer.
- Record clinical interventions and prepare pharmaceutical care plans.
- Work alongside teams to ensure safe and timely medicine supply.
- Clinically check inpatient prescriptions, leave, and discharge prescriptions.
- Provide specialist advice on medicine use to service users and carers.
- Ensure cost-effective and efficient resource use across care interfaces.
- Comply with formularies, guidelines, policies, and procedures.
- Participate in clinical audit and practice research.
- Report near misses and medication incidents.
- Assist with education and training sessions.
