Care Coordinator Wolverhampton
Key Responsibilities
- Ensuring the safety of people supported by ensuring effective deployment of care delivery colleagues.
- Undertaking all administration in line with relevant regulations and legislation.
- Managing and coordinating the care delivery team.
- Ensuring relevant documentation is in place for new people supported, discharges, and transfers.
- Conducting live call monitoring using bespoke software.
- Serving as the first point of contact for day-to-day operational requirements of care colleagues.
- Coordinating with families, healthcare professionals, and external agencies.
- Handling enquiries, scheduling appointments and meetings.
- Handling all information with confidentiality.
- Participating in the on-call rota (subject to service needs).
Skills and Expertise
- Experience in Health & Social Care
- Microsoft Office experience
- Computer literacy
- Prior experience in an administration role
- Excellent customer service skills
