Cancer Pathway Coordinator

Key Responsibilities

  • Monitor patient pathway progress and support national cancer targets.
  • Work with the Cancer Tumour Site Multidisciplinary Team (MDT) to facilitate weekly meetings.
  • Facilitate data collection for diagnosed and suspected cancer patients and enter information onto databases.
  • Monitor and facilitate patient progress on cancer diagnostic/treatment pathways.
  • Provide information to support national cancer waiting times targets.
  • Prepare and present at weekly PTL meetings with the Team Leader and ASM.
  • Act as the focal point for the MDT, ensuring smooth running and organisation of the tumour site specific MDT service.
  • Establish and maintain a register of newly diagnosed and suspected cancer patients.
  • Collate information from various sources (pathology, surgery, etc.)
  • Update cancer waiting times database for 31- and 62-day targets daily.
  • Monitor and facilitate patient progress (tracking) on cancer pathways, following escalation process to prevent delays.
  • Liaise with clinicians, nursing, and admin staff to determine reasons for waiting time delays.
  • Facilitate collection of the national cancer dataset (Cancer Waiting Times Dataset).
  • Collate data from proformas, case notes, clinics, and MDT meetings.
  • Discuss with clinicians and clinical nurse specialists.
  • Retrieve prospective and retrospective data from systems (Somerset, CERNER, etc.).
  • Collate data from referring hospitals for Cancer Waiting Times.
  • Log meeting actions and ensure they are completed.
  • Work with MDT members to ensure appropriate action and advise general practitioners of decisions.
  • Provide cross cover for other Cancer Pathway Coordinators during leave, sickness, or absence.
  • Ensure all patients on cancer pathways are treated within national target waiting times.
