Key Responsibilities

  • Work with the Lead Pharmacist to provide pharmaceutical support to Community Health Services, Newham (CHN).
  • Enhance clinical pharmacy input to Fast Falls Services (FFS) and support Extended Primary Care Teams (EPCTs).
  • Deliver Structured Medication Reviews for service users across Newham CHS.
  • Implement and monitor Trust clinical pharmacy standards, ensuring high-quality, patient-focused service.
  • Work as an integrated team member, providing medicines optimisation support.
  • Support medicines optimisation on inpatient wards.
  • Support training and education for clinical staff.
  • Support safe and effective prescribing within Newham CHS.
  • Provide input into pharmaceutical policies and procedures.
  • Provide pharmaceutical support to EPCTs and other services.
  • Participate in audits and quality improvement initiatives.
  • Provide dispensary cover (Mile End Hospital, London) as needed.
  • Deputise for CHN Lead Pharmacist in their absence.
  • Participate in pharmacy on-call service, bank holidays, and Saturday service.
  • Provide clinical pharmacy service to inpatient wards and work with the multidisciplinary team (MDT).
  • Implement and monitor Trust’s clinical pharmacy standards, ensuring high-quality, patient-focused service.
  • Support quality improvement projects.
  • Provide professional expertise, advice, support, and training to health professionals.
  • Ensure safe and appropriate use of medicines.
  • Provide clinical pharmaceutical input to Fast Falls and other services.
  • Support staff training on medicines management.
  • Organise home visits to check medication appropriateness.
  • Undertake medicines reconciliation and support one-to-one discussions.
  • Review prescriptions for accuracy and legality.
  • Ensure appropriate management of controlled drugs in domiciliary settings.
  • Monitor patients for adverse effects.
  • Ensure patient understanding of medications.
  • Liaise with community staff for effective medicine supply.
  • Support patient discharge planning regarding medication.
  • Promote self-medication for suitable service users.
  • Provide advice on unlicensed medicines use.
  • Support nurses in transcribing medications on MAR charts.
  • Manage and refer to other team members.
  • Critically appraise literature and improve medicine use.
  • Reduce risk associated with medicines use.
  • Act as a clinical role model.
  • Provide medicine information enquiry.
  • Provide accurate, evaluated, impartial, and up-to-date advice.
  • Assess medicine regimes and provide advice.
  • Identify and respond to specialist pharmaceutical information needs.
  • Communicate Trust decisions and policies to local clinical areas.
  • Support Fast Falls Services with medication review.
  • Liaise with CHN clinical leads regarding clinical pharmacy support.
  • Assist in identifying local service developments and areas for improvement.
  • Support pharmacy technicians with medicines reconciliation.
  • Ensure medicines are handled safely and securely.
  • Contribute to writing local SOPs and Trust-wide medicines policies.
  • Support local implementation of Trust Medicines policies.
  • Support in departmental and CHS meetings.
  • Assist in the preparation and delivery of training for clinical staff.
  • Participate in ward-based training for students.
  • Provide education and training for pharmacy staff.
  • Participate in ELFT Pharmacy in-house CPD sessions.
  • Participate in Trust-wide medicines audits and directorate audits.
  • Assist in identifying audit topics or project work.
  • Plan and organise workload.
  • Manage medicines storage and security.
  • Support implementing changes required by audit cycles.
  • Review and propose changes to Trust medicines policies.
  • Participate in achieving KPIs and CQUIN targets.
  • Support the Lead Pharmacist in analysing ePACT data.
  • Manage pharmacy technician involved in Newham CHS services.

Skills and Expertise

  • Clinical pharmacy skills.
  • Strong communication and interpersonal skills.
  • Knowledge of relevant legislation (Medicines Act, Mental Health Act).
  • Experience with medicines optimisation.
  • Ability to work as part of a multidisciplinary team.
  • Excellent organisational and time management skills.
  • Training experience, especially with education and training of clinical staff.
