Community Healthcare Assistant
Key Responsibilities
- Work autonomously and as part of a multi-disciplinary team to provide seamless, high-quality care to patients.
- Undertake patient care according to the needs of the patient under the supervision of a Registered Health Care Professional, adhering to Trust policies and procedures.
- Visit patients in their homes, following care plans, protocols, and policies, while respecting individual choices and maximizing patient independence and quality of life.
- Develop clinical skills and knowledge in line with Trust career pathways.
- Maintain patient dignity and respect at all times.
- Provide planned advice, support, and instruction to patients and carers, promoting understanding of treatment plans and ensuring a consistent approach to care.
- Provide domiciliary care for patients unable to manage care, and encourage patients to reach their optimal health.
- Maintain excellent communication with patients, relatives, and members of the multi-disciplinary team (MDT) regarding all aspects of care, demonstrating a variety of communication skills.
- Undertake manual handling and therapeutic handling techniques with patients, both individually and as part of a team.
Skills and Expertise
- Excellent communication skills.
- Experience with patient care.

- Proficiency in manual handling and therapeutic handling.
