Care Coordinator – Enfield Council

Key Responsibilities

  • Identifying whether support packages meet service user needs.
  • Recommending adjustments to support packages.
  • Arranging and amending support to meet needs in a person-centred way.
  • Face-to-face work with service users, families, and external partners.
  • Attending multi-disciplinary meetings and presenting cases.
  • Communicating effectively with service users, families, colleagues, and partners.
  • Writing accurate and comprehensive support plans and assessments.
  • Working with safeguarding cases and understanding safeguarding processes.
  • Working within departmental financial policies and controls, developing best value services.

Skills and Expertise

  • Excellent verbal and written communication skills.
  • Ability to assess needs and develop appropriate support plans.
  • Understanding of the care management process.
  • Working in partnership with service users, carers, and other professionals.
  • Using English to converse with members of the public, providing advice.
  • Politeness and courtesy when interacting with the public.
  • Experience working within an integrated team (Desirable).
  • Experience working with people with learning disabilities and mental health conditions (Desirable).
  • Ability to use alternative communication methods (Desirable).
  • Knowledge of relevant financial policies and controls.

Job Facilities/Benefits
