Community Learning Disability Nurse – Bromley

Key Responsibilities

  • Work in the Bromley CTLD Complex Physical Health (CPH) team, focusing on complex physical health needs.
  • Manage a caseload of adults with a broad spectrum of physical health issues, working autonomously.
  • Provide holistic, high-quality, evidence-based nursing care, assessments, and interventions.
  • Facilitate health education and health promotion training and group work with clients and carers.
  • Support students and junior staff, providing a learning experience.
  • Collaborate with a multi-disciplinary team and partner agencies/services.
  • Provide robust health liaison to acute and primary care to ensure service user access to mainstream services.
  • Demonstrate awareness of diversity and multicultural issues.
  • Apply national strategies, frameworks, and plans to meet the needs of service users regarding their physical health and wellbeing.
  • Identify and provide appropriate support for the social well-being of individual service users.
  • Work alongside the MDT and wider services to address issues for service users accessing services relevant to their health and social needs.
  • Provide health promotion relevant to individual service user needs, supporting their support network to improve health and prevent avoidable hospital admissions.
  • Liaise with mainstream services and advocate for service users needs to promote access to services.
  • Develop professional skills and knowledge through continued professional development (mandatory and relevant to the role).
  • Adhere to the NMC Code of Conduct at all times.
