Care Coordinator – Early Intervention Team
Key Responsibilities
- Responsible for a defined caseload of individuals experiencing first episode psychosis.
- Work with individuals to meet their identified needs using outcome measures and a recovery approach.
- Facilitate discharge from secondary mental health services, or referral on as appropriate.
- Contribute to duty and clinic work.
- Participate in team development issues.
Skills and Expertise
- Proven experience as a care coordinator/mental health keyworker.
- Experience in working with individuals experiencing first episode psychosis.
- Knowledge of outcome measures and recovery approaches.
- Excellent communication and interpersonal skills.
- Familiarity with discharge procedures and referral processes.
