Key Responsibilities
- Conduct comprehensive mental health assessments for older adults, considering cognitive, emotional, social, and physical health factors.
- Develop individualised care plans to address mental health needs, working in collaboration with patients, families, and other professionals.
- Participate in MDT meetings to review care plans and discuss patient progress.
- Respond to mental health crises, providing urgent support to older adults experiencing acute distress or deterioration in mental health.
- Support patients with dementia and their families in managing symptoms and planning for future care needs.
Skills and Expertise
- Degree in mental health nursing
- Fully enhanced valid DBS
- Community experience
