Early Support Coordinator – Manchester
Key Responsibilities
- Work with adults to maximise independence meeting need in creative ways.
- Deliver a timely and efficient intervention and prevent needs from escalating
- Manage pressures and demands on long term services.
- Network and build links with community assets, collaborate with partner agencies (including voluntary sector, primary and secondary health services, Childrens Services).
- Incorporate Our Manchester Behaviours into all activities.

Job Facilities/Benefits
- Full-time, permanent position
- 35 hours per week, Monday-Friday
